If you didn't pass the ASCP CG on your first attempt — first, take a breath. First: The Rules

Before planning anything else, know the constraints:

  • You must wait at least 90 days between attempts (ASCP BOC policy)
  • Maximum 4 lifetime attempts for the CG exam
  • Retake fee: $215 (vs $260 first time)
  • You can sit for retake at any Pearson VUE center, not necessarily where you failed
  • You'll see different questions — the CAT algorithm draws from a large pool

Diagnose What Actually Went Wrong

ASCP gives you a score report with performance by content area. Pull it up and identify which of these patterns matches your situation:

Pattern 1: Failed by < 100 points (a near-miss)

You knew the content but ran into test mechanics — pacing, second-guessing, anxiety. Your retake plan should be ~70% mock exams and ~30% targeted review.

Pattern 2: Failed by 100–200 points (content gaps)

You have 2–3 weak content areas dragging your score. You knew enough in some categories but not enough in others. Your plan should be ~50% targeted module review, ~50% mock exams.

Pattern 3: Failed by > 200 points (broad weakness)

You weren't ready overall — likely under-prepared on the foundations. Don't rush back in 90 days. Take 4–5 months and go back through a full study plan from the beginning.


The 4 Most Common Causes of First-Attempt Failure

After analyzing thousands of CG candidates, these are the patterns that predict a fail — and what to do differently:

1. Studying without taking timed full-length practice exams

The single biggest predictor of failure. Reading textbooks ≠ test-readiness. The CAT format is adaptive — questions get harder as you get them right, and the exam is designed to find your true skill ceiling. You need to practice at that ceiling, under time pressure, repeatedly.

Fix: Take at least 4 full-length 100-question CAT exams in your retake window. Treat them like dress rehearsals — same time of day as your real test, no breaks beyond what's allowed.

2. Memorizing flashcards without applying them

Flashcards are great for vocabulary (ISCN nomenclature, syndrome names, probe types) but the CG exam is clinical-scenario heavy. You'll see questions like "A 6-year-old presents with developmental delay; FISH shows a deletion at 22q11.2…" — not "What is DiGeorge syndrome?"

Fix: For every flashcard concept you know, force yourself to write one clinical scenario where it would appear. If you can't, you don't really know it.

3. Skipping FISH and ISCN nomenclature because they're "annoying"

These two areas are roughly 30–35% of the exam combined. Candidates who score 80%+ on these crush the test. Candidates who avoid them almost always fail.

Fix: Spend at least 30% of your retake time on FISH probe types (locus-specific, centromeric, break-apart, dual-fusion, whole chromosome paint) and ISCN structural notation. Use an ISCN cheat sheet and an interactive FISH probe trainer until both feel automatic.

4. Ignoring cancer cytogenetics

Cancer cyto is heavily weighted (often 20–25% of questions) and most first-time candidates over-prepare on constitutional cyto and under-prepare on heme malignancies. You must know the major translocations cold:

  • t(9;22)(q34;q11.2) — BCR-ABL1, CML
  • t(15;17)(q24;q21) — PML-RARA, APL (M3)
  • t(8;14)(q24;q32) — MYC-IGH, Burkitt lymphoma
  • t(8;21)(q22;q22) — RUNX1-RUNX1T1, AML M2
  • inv(16)(p13q22) — CBFB-MYH11, AML M4Eo
  • t(11;14)(q13;q32) — CCND1-IGH, mantle cell lymphoma

The 6–8 Week Retake Plan

Weeks 1–2: Diagnostic + foundations rebuild

  • Day 1: Take a full-length adaptive practice exam cold. This is your baseline.
  • Days 2–14: Review every content area you scored below 70% in
  • Use interactive training modules (not just reading) — chromosome structure, FISH probe types, ISCN basics
  • One 30-minute timed mini-quiz every other day

Weeks 3–4: Targeted weakness drills

  • Identify your 2 weakest content areas from the diagnostic
  • Spend 60% of study time on those areas
  • Daily flashcard reviews on cancer translocations and microdeletion syndromes
  • Two full 100-question CAT exams spaced across the two weeks
  • Review every wrong answer and write a one-line note explaining why you missed it

Weeks 5–6: Mock exam intensive

  • Three full timed CAT mocks in the same time of day as your real exam
  • After each mock: spend 2 hours reviewing wrong answers + adjacent topics
  • Light flashcard maintenance (15 min/day) — don't introduce new material
  • Sleep, hydration, exercise become non-negotiable

Weeks 7–8 (if needed): Polish + test-day prep

  • One final mock exam ~5 days before the test
  • Day before exam: light review only (no new material), early bedtime
  • Test day: arrive 30 minutes early, eat a real breakfast, bring water and snacks for the break

Test-Day Mechanics That Make a Difference

These small tactics have measurable impact on CAT scoring:

Don't rush the first 25 questions

The CAT algorithm sets your initial difficulty band based on early performance. Spend the time here — getting the first 25 right pushes you into a higher-scoring question pool for the entire exam.

Use the on-screen highlighter and strikethrough

Pearson VUE lets you mark choices and eliminate wrong answers visually. Eliminate even one obvious wrong choice and your odds jump from 25% to 33% on guesses.

Take the optional break

You get one scheduled 10-minute break mid-exam. Use it. Stretch, drink water, reset. Candidates who skip it consistently underperform on the back half.

Don't second-guess

On a CAT, you cannot return to previous questions. Make your best decision and move on. Anxiety-driven re-reading wastes time you'll need at the end.


Mental Game: The 90-Day Reset

Failing an ASCP exam is hard emotionally — especially after months of prep. A few things that help:

  • Don't tell everyone immediately. You don't owe anyone an explanation. Tell the people who'll support you, not the ones who'll add pressure.
  • Reframe the diagnostic information. Your score report is gold — first-time test-takers don't get this level of detail about their own weaknesses. Use it.
  • Plan the retake date within the first week. Having "May 18" on the calendar reduces anxiety more than open-ended "I'll retake eventually."
  • Tell your manager you're retaking if your job requires certification. Most labs will give you 12 months and view a planned retake more positively than ambiguity.

What If You Fail Again?

If you've now failed twice, stop and reassess before attempting #3:

  1. Are you in a cytogenetics lab actively running cases? Hands-on case experience matters enormously for this exam — far more than for most ASCP certifications.
  2. Have you taken at least 6 full-length CAT mocks? If not, you haven't built the test stamina.
  3. Are you studying solo? Find a study partner or group — verbalizing concepts to another human surfaces gaps you'd never notice alone.
